CRESTVIEW HIGH SCHOOL STUDENT PARKING PERMIT

High School student drivers can fill out the Google form to receive their parking pass. Please note you can bring in a copy of your driver's license and proof of insurance for each car, or email to pcusick@crestviewlocal.k12.oh.us   Payment can be dropped of in the high school office.
