United Way

Crestview Elementary will be hosting its 2nd annual Success By Six program here at the elementary school. Success By Six is sponsored by the United Way and truly allows the students entering into kindergarten a head start. The program will target basic skills, social/emotional needs, and allow the students to get to know the building, which will increase their overall wellbeing before entering Kindergarten. The program runs four days a week, for four weeks, beginning on July 17th from 8:00am - 11:30am, Monday through Thursday.

Final Forms

Attention

Please note, while we are working to close out the 22-23 school year, as a district we are also busy making preparations for the 23-24 school year.

One new thing that will affect our families moving forward is our change in our electronic forms vendor. We will be moving all student electronic forms from One View to a company called Final Forms.

You may have already received an email from Final Forms asking you to complete your child's profile for the 23-24 school year. You will need to create an account associated with your email address. You can then update all necessary school forms and documents from your new account.

If you have any issues, please feel free to reach out to the Administration office at 330-482-5526, option 1 and ask for Jessica or Christy.

Thank you for your understanding and patience as we make this transition.

23-24 New Student Enrollment

Enrollment

New student enrollment is now open for the 2023-2024 school year! To start the application process please visit our Final Forms new student enrollment site located here.

We are currently accepting new kindergartners who will be age 5 by September 30th as well as applications for new open enrollment students. For more information please call 330-482-5526, option 1.

Crestview High School Community Service Forms

Crestview High School Community Service

Students at Crestview High School are encouraged to complete 25 hours of community service hours each year, for a total of at least 100 at the time of graduation. By doing so, students can earn 1/4 of a credit each year totaling 1 credit community service on their transcripts upon graduation.

We have put the form online for your convenience. The form should be filled out entirely with no fields left blank.

If you have any questions, please email the high school principal or high school counselor for more information.
